PHOTO & VIDEO RELEASE
Without expectation of my compensation or other renumeration, now or in the future, I hereby give my consent to FLOUANDNOU BEAUTY and its representatives to take photographs and/or video footage of me for the purposes of documenting my semi-permanent makeup procedures.
I understand that these photographs and/or videos may be used for the following purposes:
(a) My personal clinical records
(b) Training purposes for FLOUANDNOU Beauty staff
(c) Marketing and promotional materials for FLOUANDNOU BEAUTY, including but not limited to website, social media, print ads, etc.
This consent is given in perpetuity, and does not require prior approval by me.
I reserve the right to request removal of any published photos and videos in which I am featured.
